Focused Shockwave Therapy: A New Approach to Pain Relief and Healing

Estimated reading time: 3 minutes
If you've been dealing with persistent pain from an old injury, tendonitis, or a condition that just doesn't seem to improve, focused shockwave therapy may offer another option.
Focused Extracorporeal Shockwave Therapy (fESWT) is a noninvasive treatment that uses high-energy acoustic waves to stimulate the body's natural healing processes. Originally developed in Germany from technology used to treat kidney stones, shockwave therapy is quickly becoming a popular treatment for chronic musculoskeletal conditions in the United States.
How Does Focused Shockwave Therapy Work?
Despite its name, shockwave therapy has nothing to do with electricity. Instead, it uses precisely directed sound waves that travel through the body's tissues.
These waves produce mechanical stimulation that can trigger biological responses associated with tissue repair, circulation, and pain regulation. Researchers believe these effects may help stimulate healing in tissues that have struggled to recover on their own.
Unlike radial pressure wave therapy, which delivers energy primarily to more superficial tissues, focused shockwave therapy allows energy to be concentrated at a selected depth, making it useful for targeting specific structures beneath the skin.
What Conditions Can Shockwave Therapy Treat?
Focused shockwave therapy has been studied for a variety of musculoskeletal conditions, particularly chronic tendon problems.
Common applications include:
Plantar fasciitis: Persistent heel pain, especially with the first steps in the morning.
Tennis elbow: Pain on the outside of the elbow, often aggravated by gripping or lifting.
Achilles tendinopathy: Pain and stiffness in the tendon connecting the calf to the heel.
Patellar tendinopathy: Pain below the kneecap, often associated with running and jumping.
Calcific shoulder tendinopathy: Pain associated with calcium deposits in the shoulder's rotator cuff tendons.
Greater trochanteric pain syndrome: Pain along the outside of the hip, often aggravated by walking or lying on that side.
Research supports shockwave therapy as a treatment option for several of these conditions, but of course, its effectiveness varies depending on the diagnosis and individual patient.
What Should I Expect During Treatment?
A focused shockwave session is relatively quick and requires no injections or incisions.
Your provider applies a coupling gel to the treatment area and uses a handheld applicator to deliver a series of acoustic pulses into the targeted tissue.
You may experience a tapping sensation and mild to moderate discomfort during treatment. The intensity can be adjusted to improve comfort.
Most sessions take approximately 10–20 minutes, depending on the area being treated. A series of 6-12 treatments is often recommended, typically spaced about a week apart.
Some patients notice early pain relief, while others experience gradual improvement over the following weeks as the body responds to treatment.
Temporary soreness, redness, or bruising can occur. Because shockwave isn't appropriate for every condition or patient, a clinical evaluation is important before beginning treatment.
Is Shockwave Therapy Right for You?
One of the greatest advantages of focused shockwave therapy is that it offers a noninvasive treatment option without surgery or injections.
However, shockwave therapy isn't a cure-all. It is often most effective as part of a comprehensive treatment plan that may include therapeutic exercise, activity modification, and rehabilitation.
At PALM Health Center, we believe that addressing the underlying factors contributing to pain is just as important as providing relief.
